There are several forms that the government requires when a new employee is hired. Even though you are running your placement as your own company, you will need to fill out this information for us.

• W-4 Form - Employee Withholding Election
• I-9 Form - Immigration & Naturalization Certification

Please download these forms from http://www.esmartpayroll.com/FedPayroll.htm print, and complete. If you do not have that capability, please contact us and we will mail or fax them to you for completion.

These forms must be in our possession before you begin work. In addition to the I-9 form, the valid forms of identification as specified must be photocopied and included for our records.
